Medicines reconciliation – is there a best practice to obtain correct medicines lists at admission to hospital?(20 minutes presentation on 21/02/2018

Studies have shown that for up to 80% of patients there is one or more discrepancies between the list of medicines obtained at admission and the patient´s actual list of medicines. Many approaches have been developed aimed at obtaining correct lists of medicines.

Objectives:

  • know what medicines reconciliation is
  • be familiar with the most prevalent approaches to medicines reconciliation
  • be familiar with the strengths and weaknesses of these approaches
  • be able to reflect on which elements to address in order to develop a best practice of medicines reconciliation in their own settings
