ROLE OF COMMUNITY PHARMACIST IN IDENTIFYING AND RESOLVING DRUG THERAPY PROBLEMS

Community pharmacists are the most accessible healthcare providers besides being the last healthcare provider encountering patients before they use their medicine. Drug related problems (DRPs) have the potential to increase patients’ morbidity and mortality. For this, pharmacists mainly as the drug experts carry the responsibility of assuring rational drug use along with other cognitive functions; dispensing, patient education, prevention and early detection of diseases.

This is achieved by providing pharmaceutical care services that assure drug therapy appropriateness, patient understanding, medications availability & preparedness, provide drug information, health promotion & disease prevention services and finally by medication therapy management.

The aim of this activity is to describe the most common DRP associated with different pathologies and encountered in community pharmacies, along the ways to resolve them on different levels. Also we cover essential monitoring of common medicine and ways of communication with other healthcare providers to assure efficacy and early detection of drug therapy problems (DTPs).

Objectives:

  • Identify and prioritize DRPs including adverse drug events, drug interactions, and/or suboptimal treatment.
  • Effectively communicate DRP with patients and other healthcare professionals as necessary.
  • Plan for strengthening clinical skills necessary to DRP detection and management and implementing programs to individualize pharmaceutical care plans in their own settings.
