WHAT ARE THE WEAKEST LINKS IN THE MEDICATION PROCESS? HOW SHOULD THEY BE MANAGED?

The medication process constitutes many risks of making mistakes. To mitigate risk to patients and health personnel there are in most settings systems in place which aim to make it possible to do the right things in the correct way. Nevertheless, the ideal system is yet to be developed, and in many settings there is work to do to improve the safety culture of the staff.

Objectives:

  • know the elements of the medication process
  • know an example of a medication process system with its inherent strengths and weaknesses
  • know the elements of a good safety culture
  • be able to reflect on the quality of the medication process in their own settings in terms of strengths and weaknesses, and how to develop their system towards optimal safety.
